On cook line butter (106F - Reheating) for 2 hours. Spoke with operator regarding rapid reheating within a 2 hours perimeter, operator discarded butter and began reheating a new batch using a rapid reheat method in microwave.

Operator iced down both foods, temperature in 15 minutes under 41F. Repeat violation. Operator is covering product and has moved cooler away from grill, discussed with operator using ice to aid in cooling due to proximity to cook line, monitoring temperatures, as well as consulting a technician about cooler.
No paper towels or mechanical hand drying device provided at handwash sink at drive through window. Operator placed paper towels at sink.

Dishwasher (Chlorine 0ppm) after running and priming three times. Operator set up manual sanitation in triple sink, quaternary 200ppm.
Single-use gloves not changed as needed after changing tasks or when damaged or soiled. Observed cook handling raw ground beef burgers then begin to fill ketchup bottles, no glove change no handwash. Spoke with manager, manager discussed with employee who changed gloves and washed hands before returning to work.

Operator iced down foods for quick cooling. This is a repeat violation from 11/1/2023. Discussed with operator proper cold holding procedures and time as a public health control.
Food being cooled by nonapproved method. In walk-in cooler observed cut tomatoes ambient cooling, packed densely in covered, deep, plastic container. Spoke with operator about proper cooling techniques to aid in proper ambient cooling.
