Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit. Observed custard mix at 11:15 (51F - Cold Holding) the machine was in the off position. An employee stated the custard machine was filled approximately 15 minutes prior to the temperature check. The machine was turned on temperature for the custard mix at 11:37 (39F - Cold Holding). Observed cooked chicken (50F - Cold Holding) stored stacked above the fill line in the top section of the reach in cooler near the grill. The operator emptied half of the contents and stored in the walk in cooler. Observed butter packet (58F - Cold Holding) in the counter top cold holding unit behind the front counter. The cooler was not turned on. An employee stated the potentially hazardous foods had been held for approximately 1 hour. The employee stored the butter in the lower portion of the cooler.
Food stored on floor. Observed boxes of French fries stored on the floor in the walk in freezer.
No conspicuously located ambient air temperature thermometer in the counter top cold holding unit behind the front counter.

In-use utensil not stored with handle above the top of time/temperature control for safety food and rim of the container. Observed tong handles used for raw ground beef stored touching the raw ground beef in the reach in cooler near the grill. The operator removed the tongs.

The operator stated the cod has been held for approximately 1 hour. The operator removed half the contents and stored in the walk in cooler. Observed cooked chicken (52F - Cold Holding); cooked corned beef (49F - Cold Holding) in the reach in cooler near the grill. The operator stated the foods have been held for approximately 1 hours. The operator moved the above foods to the walk in cooler.
