Per operator item was reheated on stove and placed in steam table for hot holding. Item stored in water in shallow pan above water of steam table, operator switches pan to deeper pan and reheated to 165F

Single-use gloves not changed as needed. After touching a dirty towel and cleaning a cutting board with gloves employee proceeded to plate a customer order without removing gloves and washing hands. Instructed employee to wash hands, employee washed hands.
Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit. Prep table: shredded cheese (67F - Cold Holding). On prep table less than 4 hours. Observed stored at room temperature. Operator moved to freezer to quick chill. Foods not prepared or portioned today.
Food being cooled by nonapproved method as evidenced by inadequate rate of cooling during time of inspection. 1. Flip top by grill: cooked vegetables 135F at 9:45am cooling for 15mins at 11:00am 98F. Observed cooling at room temperature less than 4. Operator moved to walk in cooler for quick chill. 2. Dice tomatoes 62F at 10:15am cooling since 8am 52F at 11:00am slice tomatoes 51F 10:15am cooling since 8am to 11:00 to 50F. Operator moved to walk in cooler for quick chill.
