Storage area not maintained clean and organized. Storage room at back exit with excessive miscellaneous items/boxes to door area. Also, unused microwaves and flip top reach in cooler stored next to back exit. Manager states they are waiting for repairs to be done.

Ceiling/ceiling tiles/vents/walls/floors soiled with accumulated food debris, grease, dust, or mold-like substance. Vent above ice machine, vent above drive thru slush machine, vent at dessert prep table and vent in men's room has accumulation of dust. Ceiling tiles soiled at front counter and at lobby reach in freezer.
Dead roaches on premises. One dead roach observed in far left corner of water heater. Employee removed and discarded dead roach.
Exterior door has a gap at the threshold that opens to the outside. Gap at bottom with visible outside light coming through at rear exit door.
Floor tiles missing and/or in disrepair. Multiple floor tiles in walk in freezer along right side missing.
Lights in food preparation, food storage or warewashing area missing the proper shield, sleeve coatings or covers. Unshielded lights above three compartment sink.
