Ceiling vents were soiled with accumulated dust and/or a mold-like substance. Two vents were located at the front counter, three in the dining area, and two in the ice cream service area.

Floor soiled/has accumulation of debris. The floor on the left side of the ice bin in the dry storage area is soiled with slime.

Unwashed onions stored in crate in walk-in cooler ready-to-eat opened bag of hot dogs and bacon. Operator moved and stored correctly.

Plumbing system in disrepair. Observed standing water on floor in front of 4 door reach-in freezer in front of office. Operator stated water was from the drain near ice cream machine that had clogged. Operator disconnected all sources of water from area and plans to unclog the drain shortly. Area has been cleaned and sanitized and no more water is observed in area.

Paper towel dispenser at handwash sink not working/unable to dispense paper towels at handwash sink in women's bathroom. Operator made paper towels accessible. New dispensers have been ordered.
Accumulation of black/green mold-like substance in the interior of the ice machine and lid.
Equipment and utensils not properly air-dried - wet nesting on containers stack together to the left of the three compartment sink on shelf.
Equipment in poor repair. Observed white chest freezer to the left of the fryers bubbled, cracked, and rusted on the side.
