Observed Stephanie Galvan ( 1 month) has not signed the food employee reporting agreement. Manager provided the form to employee and it was signed.

No proof of required state approved employee training provided for any employees. To order approved program food safety material, call DBPR contracted provider: Florida Restaurant and Lodging Association (SafeStaff) 866-372-7233. Observed Elizabeth, Jailyn, Fer, Miranda, Issa, Adriana on schedule for more than 2 months but not Food handler training provided.
