Toxic substance/chemical improperly stored. Observed : Spic and Span spray at the wait station stored next to clean plates. This was moved to cleaning. ALSO, at the bar, two spray bottles of sanitizer hung on the speed rail with bottles of alcohol. These were moved to cleaning.
Food-contact surface soiled with food debris, mold-like substance or slime. Observed multiple bar soda guns with buildup. ALSO, the can opener blade has dried food debris on it.
Handwash sink used for purposes other than handwashing. Observed : one bar hand wash sink used to store wd 40. This was moved to cleaning.
No paper towels or mechanical hand drying device provided at handwash sink. Observed : lack of paper towels at a bar hand wash sink.
Required employee training expired for some employees. To order approved program food safety material, call DBPR contracted provider: Florida Restaurant and Lodging Association (SafeStaff) 866-372-7233. Observed : 1 expired training certificate.
Dishware/utensils not soaked prior to being placed in three-compartment sink/dishmachine and results in soiled dishware/utensils after washing occurs. Observed : cheese debris on a plate for service at the wait station. This was moved to dish.
Food stored on floor. Observed : bulk cases of food on the floor in the walk-in freezer.
Hole in or other damage to wall. Observed : the two walls around the mop sink are deteriorating.
In-use tongs stored on equipment door handle between uses. Observed tongs on the cook line protruding into the walkway. These were turned sideways.
In-use utensil stored in standing water less than 135 degrees Fahrenheit. Observed spatulas on the cook line in a pan of standing water at 114°.

Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit. Observed: on the cook line, chicken bread area chicken 47, fish 51, chicken 55. These items were brought from the walk-in cooler to stock after lunch. Theses items were iced down and were reaching 41F in 18 minutes. Warning* ALSO, the reach-in cooler Thai chili sauce 44F, cut tomato 57(cut after lunch), tomato mix 47. All items were originally in plastic food pans, and due to lack of spacer bars were double panned. Other time/temperature control for safety items were at 42° and below. Icing was being accomplished at this unit also.

Bowl or other container with no handle used to dispense food. Observed: soufflé cup used to portion on the cook line.
Hole in or other damage to wall. Observed the wall at the mop sink is comprised and has a hole.

Unwashed fruits/vegetables stored with ready-to-eat food. Observed: avocados on the cook line with the product sticker still attached. These were moved back to prep.

This was placed in the walk-in cooler. ALSO, chicken portioned in bags earlier today, 45F, on the cook line. These were placed flat on a tray and moved to the walk-in freezer.
