Lack of toilet tissue at each toilet. Observed : in the women's restroom, lack of toilet paper in the far stall.

No soap provided at handwash sink. Observed : the soap dispenser at the hand wash sink in the kitchen would not dispense soap, it was dried in place soap. Dish soap was used to wash hands.

Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit. Observed: butter chips in a freezer food pan, on the counter in the kitchen, 62F. These had been out for service earlier, starting at 6 am.
Toxic substance/chemical improperly stored. Observed: dog carpet shampoo stored over to go single serve in the dry storage room. The shampoo was moved to cleaning.
Food-contact surface soiled with food debris, mold-like substance or slime. Observed: the reach-in freezer in the Market has spills on the bottom shelf.
No chemical test kit provided when using sanitizer at three-compartment sink/warewashing machine or wiping cloths. Observed: lack of a chlorine test kit.
No probe thermometer provided to measure temperature of food products. Observed: lack of a probe thermometer.
No proof provided that food employees are informed of their responsibility to report to the person in charge information about their health and activities related to foodborne illnesses. Observed the front desk staff that handled the food service this morning lacks the Employee Reporting form. A blank copy was already available in an email.
No sanitizer of any kind available for warewashing. Only use single-service items to serve food to customers until sanitizer is available for warewashing. Observed: the kitchen lacked sanitizer. Chlorine bleach was brought from the laundry.
Ice buildup in reach-in freezer and/or walk-in freezer. Observed: ice building in the dining area reach-in cooler, back wall is frozen over.
