Employee failed to wash hands before putting on gloves to initiate a task working with food. Observed employee changed gloves without washing hands first. Coach, employee and proper handwashing procedure then she proceeded to wash hands and put on new gloves.
Wiping cloth sanitizer solution exceeds the maximum concentration allowed. Observed chlorine sanitizer bucket at 200plus PPM. Employee diluted solution with a final reading of 50 ppm.
Food-contact surface soiled with food debris, mold-like substance or slime. Observed knives soiled with food stored on pizza oven.
Ice buildup in reach-in freezer and/or walk-in freezer. Observed ice buildup at reach in freezer.

No probe thermometer provided to measure temperature of food products. Observed no probe thermometer at time of inspection.
