Ceiling/ceiling tiles/vents soiled with accumulated food debris, grease, dust, or mold-like substance. Upon inspection observed multiple ceiling tiles damaged throughout restaurant.

Observed multiple food items held above 41F in the following locations: Front Counter Reach in Cooler: homemade ranch 55F, homemade caesar dressing 50F Kitchen Counter: garlic parsley oil 78F at 1:30. Per employee ranch and caesar dressings held since yesterday. See stop sale. Per manager garlic parsley oil held since 11:00. Manager iced garlic parsley oil during inspection. Garlic parsley oil 55F at 1:45.
Menu does not identify which items contain raw or undercooked animal foods covered by the consumer advisory. Per manager homemade caesar dressing contains raw egg yolks. Observed caesar salad not indicated as undercooked product with consumer advisory on menu.
