Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit. Observed cut tomatoes (54F - Cold Holding); cut lettuce (57F - Cold Holding) left on counter per person in charge for 30 mins. Person in charge placed in refrigerator as inspector arrived.
Vacuum breaker missing at hose bibb or on fitting/splitter added to hose bibb. Observed hose Bibb used to fill fresh water tank missing back flow preventer.
An MFDV that connects to an approved water system must also connect to an approved wastewater system to prevent potential overflow of the wastewater holding tank. Observed MFDV connected to potable water tank without being connected to waste water.
Establishment has no written procedures for employees to follow in response to a vomiting or diarrheal event where the vomit or diarrhea is discharged onto surfaces in the establishment. Provided written procedures and discussed with person in charge.

All time/temperature control food for safety stored outside of the trailer and not at the commissary location when the mobile food truck is not in use per operator.
