Food-contact surfaces not sanitized after cleaning, before use. Do not use equipment/utensils not properly sanitized. As per operator they only washed the utensils, blenders and containers.
Nonfood-grade bags used in direct contact with food. Observed several fruits pulp stored in thank you bag inside and outside of white reach in freezers. Operator was informed that the bags need to be changed.

Establishment has no written procedures for employees to follow in response to a vomiting or diarrheal event where the vomit or diarrhea is discharged onto surfaces in the establishment. Clean up of vomiting and diarrheal events was provided.

Handwash sink removed from food preparation/dishwashing area. Must be reinstalled in the same location where removed. Observed the hand wash sink as per the plan review located next to three compartment sink was removed.
