Food-contact surface of silverware provided for customers at salad bar/buffet not properly protected. Observed self-service plastic forks stored with the food contact surface up at the counter next to the soda machine in the lobby. The operator voluntarily discarded the plastic forks.

Food-contact surface of silverware provided for customers not properly protected. Observed unwrapped plastic forks stored with the food contact surface up near the soda machine in the lobby. The operator voluntarily discarded the forks.
Ice buildup in reach-in cooler in the center of the kitchen.

Observed employee touching face and visor, then resumed work. Observed operator coaching, explaining, and employee washed hands properly.
Handwash sink used for purposes other than handwashing. Observed on front line hand sink, a bucket of water and plastic inside sink. Observed operator removing items and cleaning sink.
Equipment in poor repair. Observed torn gasket at grill station reach ins.
Nonfood-contact surface soiled with grease, food debris, dirt, slime or dust. Observed soiled gaskets on reach in cooler on line. Observed employee cleaning gaskets.
Stored food not covered. Observed seasoning in a cambro with no cover on it. Observed operator volunteerly throw away seasoning.
Working containers of food removed from original container not identified by common name. Beer Eva's Daily Report rice in cambros with out identifying label. Observed operator labeling rice.

Food-contact surface soiled with food debris, mold-like substance or slime. Observed Cheese shredder put away clean with dried cheese in different places of shredder. Observed operator putting shredder into dish and cleaning shredder.
Handwash sink used for purposes other than handwashing. Observed hand washing sink next to coffee and tea machines being used as a dump sink.
Buildup of food debris/soil residue on equipment door handles. Observed on reach in coolers on make lines, food debris on back side of handles.
Floor not cleaned when the least amount of food is exposed. Observed throughout make area, storage area, underneath sinks food build up along cove molding. Observed operator starting to clean.
Nonfood-contact surface soiled with grease, food debris, dirt, slime or dust. Observed through out kitchen, make area, gaskets with food debris on them.
Old food stuck to clean dishware/utensils. Observed build up on lime slicer. Observed operator rewashing lime slicer.
Old labels stuck to food containers after cleaning. Observed an old label on large white bin. Observed operator removing label and having bin re washed.
Outer openings not protected with self-closing doors. Observed back door self closure disengage due to being damaged. Operator stated that new self closure has been ordered.
Reach-in cooler interior/shelves have accumulation of soil residues. Observed on make line reach in coolers with food debris build up on bottom of cooler.
