Also, in same cooler, raw chicken and raw pork chops over containers of tomatoes and blended peppers. Employee began to rearrange cooler.
Shell eggs not stored at an ambient air temperature of 45 degrees Fahrenheit or less. Counter top reach in cooler on cook line, raw shelled egg (49F - Cold Holding). Employee stated eggs have been in cooler since yesterday. Ambient air of reach in cooler 51F.
Stop Sale issued due to food not being in a wholesome, sound condition. In stand up reach in cooler near entrance to kitchen, four packs of raw salmon thawed in vacuum sealed packaging.
Stop Sale issued on time/temperature control for safety food due to temperature abuse. Counter top reach in cooler on cook line, raw shelled egg (49F - Cold Holding). Employee stated eggs have been in cooler since yesterday. Ambient air of reach in cooler 51F.
Handwash sink removed from food preparation/dishwashing area. Must be reinstalled in the same location where removed. Handwash sink on outside of entrance door to kitchen removed and plastered over.

In stand up reach in cooler near back door, 1 pan of raw shrimp, 1 pan of raw salmon and 1 pan of raw tilapia stored on shelf above commercially packaged blocks of queso cheese and 1 bag of shredded cheese. Also, in stand up reach in cooler near back door, large container of thawing raw pork sausage on shelf above containers of sour cream, cut tomatoes, open bag of shredded cheese and pan of cooked chicken.
Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit. In flip top reach in cooler on cook line, salsa (45F - Cold Holding), raw pork skin/meat (48F - Cold Holding). Salsa was thawed out this morning and put in reach in cooler with lid open.
Time/temperature control for safety food, other than whole meat roast, hot held at less than 135 degrees Fahrenheit. In steam table, beef soup with corn on cob (130F - Hot Holding). Cook stated it had been in steam table for approximately 1 hour. Cook turned steam table up to help bring temperature up. Rechecked approximately 20 minutes later at 156F.
Food-contact surface soiled with food debris, mold-like substance or slime. Red and green cutting boards on prep table stained black.
Accumulation of black/green mold-like substance in the interior of the ice machine/bin. Interior of ice machine soiled with pink/black mold-like substance.

Manager moved raw shrimp to other side of reach in freezer with other raw fish products. Also, in reach in freezer to right of kitchen entrance, plastic bags of raw pork stored on top of commercially packaged vegetables. Also, open bag of raw chicken stored on top of commercially packaged vegetables. Employee began moving products.
Raw animal food stored over/not properly separated from ready-to-eat food. In reach in freezer at end of cook line next to stove, pan of raw beef stored on shelf above pan of ready to eat baked beans. Manager moved baked beans to top shelf of reach in cooler.
Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit. In reach in cooler at end of cook line by back door, salsa (45F - Cold Holding). Per manager employee had bucket of salsa put to fill cook line then placed back into reach in cooler.
Food service manager not certified within 30 days of employment.
