Unit door was slightly open and has a gap. Ambient temperature reading at 42F. Manager place items in reach in freezer to bring down temperature. All items re-temp at 41F or below.

Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit. Observed milk (49F - Cold Holding) in reach-in glass cooler at front service line. Observed two shelves above milk full causing no air flow throughout cooler. Operator rearranged cooler to provide better air flow and moved milk to top shelf near fan. Also, house made bread pudding (44F -Cold Holding)in front display case. Operator stated employee left door open on display case. Rechecked temp at end of inspection and bread pudding was 43F and milk at 46F.
Time/temperature control for safety food, other than whole meat roast, hot held at less than 135 degrees Fahrenheit. Observed cooked pork belly, beef empanadas (127F-129F - Hot Holding); in display food warmer at front counter. Operator stated employee left door open during lunch rush. Operator raised temp of food warmer. And placed pork belly and empanadas in oven to reheat to 165F. Retemped both foods at end of inspection at 165F-166F
Food-contact surface soiled with food debris, mold-like substance or slime. Observed underside of shake machine at front service line and underside of mixer in prep area soiled. Employee started to clean mixer during this inspection.
Handwash sink used for purposes other than handwashing. Observed hand wash sink next to triple sink used to store a pan. Operator removed pan from hand wash sink.
No paper towels or mechanical hand drying device provided at handwash sink. Observed no paper towels at hand wash sink in men's restroom. Operator replenished paper towels.

Time/temperature control for safety food, other than whole meat roast, hot held at less than 135 degrees Fahrenheit. chicken empanadas, cooked sausage (108-114F - Hot Holding) in front hot box display. Items in hot box less than 1 hour. Operator raised temp of box to reheat.
Wiping cloth sanitizer solution exceeds the maximum concentration allowed. Sanitizer Bucket (Quaternary +500ppm-strip turned blue ) at front service line. Operator added more water to dilute sanitizer solution. Retested at 400ppm.
Paper towel dispenser at handwash sink not working/unable to dispense paper towels. Paper towel dispenser at hand wash sink nearest mop sink unable to dispense paper towels. Operator placed a roll of paper towels near hand wash sink until batteries get changed in dispenser.
Water with a temperature of least 85 degrees Fahrenheit not provided/shut off at employee handwash sink. No hot water at hand wash sink nearest mop sink.
Equipment in poor repair. Gasket on 1st and 3rd door of stand up reach-in cooler in prep area torn.
Food stored on floor. Open Box with sealed bags of coffee at front service line, sealed bag of coffee beans in back dry food storage area, and opened box with sealed sleeves of ice cream cones in dry food storage room stored on floor. Operator removed foods to properly store.

Observed ice build up on fan. Operator stated both foods were placed in cooler today. Operator removed both TCS foods and placed in nearby reach-in cooler to chill.

Cold holding equipment not maintained in good repair. Do not store time/temperature control for safety food in this unit until the unit is repaired. Reach-in cooler dessert display case at front counter unable to maintain temperature of 41F. Ambient temperature 48F. Operator had foods removed and placed cooler into defrost mode to melt ice during this inspection.
