Cove molding at floor/wall juncture broken/missing. Multiple areas thru the food storage and prep areas were covered molding/base boards missing and/or broken.
Floor not cleaned when the least amount of food is exposed. Floors, walls, baseboards and ceiling tiles soiled all thru the location
Floor tiles missing and/or in disrepair. Multiple areas were tiles tiles broken and or missing.
Interior of refrigerator or freezer in disrepair/has exposed insulation. Multiple reach in cooler drawers in disrepair, not maintaining temp of 41° or colder, currently not holding any food items at this time.
Nonfood-contact surface soiled with grease, food debris, dirt, slime or dust. Exterior of equipment on cooks line heavily soiled, old food and grease. Multiple door handles soiled on cooks line.

Per managers log from 6am all items in reach in cooler unit at 38-39° cold holding. All items pulled to walk in freezer to rapidly chill within 4hour mark, Rechecked temps in walk in freezer raw tbone steak surface temp 40-38° internal temp44°: Raw sirloin steaks 41° cold holding in walk in freezer.
Time/temperature control for safety food, other than whole meat roast, hot held at less than 135 degrees Fahrenheit. Commerically prepared mashed potatoes hot holding at 103-114° for approx 1 hour, placed back into microwave and reheated to 200° within 10 mins.
