Ready-to-eat, time/temperature control for safety food marked with a date that exceeds 7 days after opening/preparation: observed some of the time and date stickers that are automatically created have the current date and then eight days instead of the required seven (example: item dated for 08/06/2025 has a use by date of 08/13/2025. Explained to General Manager about the discrepancy. He will reach out to their company to get it adjusted correctly).

Handwash sink used for purposes other than handwashing: observed the cook on the cook line wash a frying pan in the hand-wash sink.
Wiping cloth quaternary ammonium compound sanitizing solution not at proper minimum strength: observed wiping cloth solution at wait station tested to 0ppm. General Manager made new solution. Retested to 200ppm.

Dishmachine chlorine sanitizer not at proper minimum strength. Discontinue use of dishmachine for sanitizing and set up manual sanitization until dishmachine is repaired and sanitizing properly: Dish machine sanitizer tested to 0ppm. General Manager had employee setup third sink of three compartment sinks and sanitize the dishes there. three compartment sink, quaternary ammonium, 200ppm.
Ceiling/ceiling tile shows damage or is in disrepair: Ceiling in front part of dining room (west side of the building) leaking in five places. This area of the dining room is shut down and customers are not seated there.

Single-use gloves not changed as needed after changing tasks or when damaged or soiled: Cook walked away from the cook's line wearing gloves. Upon return, he was still wearing the gloves. Assistant General Manager gave corrective feedback to the cook. He removed his gloves, washed his hands correctly, dried his hands, and then donned new gloves.

Food stored on floor: - box of lemons and leaf lettuce, crate of onions and iceberg lettuce on floor in walk-in cooler. Assistant General Manager immediately put on shelves; - box of marinara on floor in walk-in freezer. Assistant General Manager immediately put on shelf.
Water draining onto floor surface: - drain pipe under hand-wash sink not connected correctly allowing for water to drain onto floor. Assistant General Manager states that maintenance has been notified.
