Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit. Milk in carton in flip top reach in cooler at 45F. Employee states milk was left out on the counter for approximately 10 minutes. Milk placed in reach-in freezer and temperature taken approximately 10 minutes later at 40F.
Equipment in poor repair. Right door gasket on stand up reach-in cooler is torn.
Single-service articles improperly stored. Takeout containers stored on the floor by restroom. Employee moved containers to shelving.

Wiping cloth chlorine sanitizing solution not at proper minimum strength. Wiping cloth solution by triple sink tested at 0ppm quaternary ammonium. Employee remade solution to 200ppm quaternary ammonium.

Ready-to-eat, time/temperature control for safety food not consumed/sold within 7 days after opening/preparation. See stop sale. In reach in cooler by hot dog warmer, package of hot dogs opened eleven (11) days prior. Hot dogs discarded.
Vacuum breaker missing at hose bibb or on fitting/splitter added to hose bibb. Breaker missing on outside faucet in dumpster area where black hose is attached.
Establishment has no written procedures for employees to follow in response to a vomiting or diarrheal event where the vomit or diarrhea is discharged onto surfaces in the establishment. No written plan available. Inspector emailed Form 5030-104 (Clean Up Instructions for Vomitting/Diarrhea).
Establishment did not report seating change that affects the license fee, Clean Indoor Air Act, sewage system approval or other related requirements. Establishment has thirty-eight seats including indoor and outdoor seats, but establishment is only licensed for ten (10). Inspector sent change of record form to change seat number and emailed establishment Seating Change Evaluation Form.
Floors, walls, and/or ceilings soiled/has accumulation of debris. Ceiling vent in back dry storage area is dusty.
