Raw animal food stored over or with ready-to-eat food in a freezer - not all products commercially packaged. Open bag of raw beef for philly's sandwiches stored on top of boxes of ravioli and cannoli cream in walk in freezer, beef moved.
Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit. sliced cheese (55F,57F,53F - Cold Holding); mortadella (51F - Cold Holding); shredded lettuce (48F - Cold Holding); cut tomatoes (48F - Cold Holding) in top section of reach in cooler on wall next to storage area with missing containers in top section, per operator lid and doors have been consistently open for several hours, operator began to move items around during inspection and adjusted temp of cooler.
