Live, small flying insects found. In back storage area, 11 on walls and equipment around carbon dioxide tanks, 5 on boxes above prep sink. In bar area a total of 75 observed - locations are 15 on wooden border of ceiling, 20 on hanging light fixtures, 35 on white veiling tiles above bar, 5 hovering around far end right side soda gun. None landing on food, food contact surfaces or on main cook line.
Ceiling/ceiling tiles/vents soiled with accumulated food debris, grease, dust, or mold-like substance. Ceiling in main kitchen, at door to walk-in freezer, by vent in walk-in cooler with accumulated dust. Ceiling tiles in bar area heavily soiled around edges, dust on attached lighting.

Employee on grill touched raw ground beef then cooked chicken, no glove change or handwash. Spoke with employee, employee changed gloves and washed hands properly. Chicken was heated on grill.
