Employee personal items stored in or above a food preparation area, food, clean equipment and utensils, or single-service items. There is a cellphone on a clean plate by the triple sink. Operator removed the cellphone and moved the plate to the dish washing station.

Dishmachine not sanitizing properly. Discontinue use of dishmachine for sanitizing and set up manual sanitization until dishmachine is repaired and sanitizing properly. Dishwasher (Temperature 149F), operator set up the triple sink, quaternary sanitizer 200ppm.
Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit. In the drawer cooler under the grill Fish (45F - Cold Holding). Operator moved the fish yo the walk in cooler.
Food-contact surface soiled with food debris, mold-like substance or slime. There is food debris behind the slicer blade and on the mixer head. Operator cleaned both the slicer and the mixer.
