Food-contact surface soiled with food debris, mold-like substance or slime. There is dried food debris on the guard around the slicer blade and on the can opener blade. The can opener was moved to the dish washing station, the slicer was cleaned and sanitized.
Accumulation of black/green mold-like substance in the interior of the ice machine/bin. There is a mold like substance on the ice chute in the ice machine.
Ice buildup in reach-in freezer and/or walk-in freezer. There is a buildup of ice around the walk in freezer door.

Container of medicine improperly stored. There is a bottle of vis drops there is a bottle is a squirt bottle of visine stored next to clean plates. Operator removed the visine.
Dishmachine not sanitizing properly. Discontinue use of dishmachine for sanitizing and set up manual sanitization until dishmachine is repaired and sanitizing properly. Dishwasher (Temperature 149F), operated set up the triple sink with quaternary sanitizer 200ppm.
Raw animal food stored over/not properly separated from ready-to-eat food. In the reach in cooler cold smoked salmon is stored above cooked chicken and heavy cream. Operator moved the cold smoked salmon to proper storage order.
Proof of required state approved employee training not available for some employees. To order approved program food safety material, call DBPR contracted provider: Florida Restaurant and Lodging Association (SafeStaff) 866-372-7233. There is no proof of training for any servers.
In-use utensil stored in standing water less than 135 degrees Fahrenheit. In the triple sink there are utensils in 77F standing water. Operator moved the utensils in water to the flat top grill.

Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit. On a prep table next to the grill, roast beef (49F - Cold Holding). Per operator the roast beef had been there less than 30 minutes. Operator moved the roast beef to the freezer for rapid cooling.
Toxic substance/chemical improperly stored. There is a spray bottle of disinfectant cleaner stored next to croutons and a container of hand soap stored next to a squeeze bottle of oil by the triple sink. Operator removed the cleaner and soap.
Food service manager not certified within 30 days of employment. Handwash sink not accessible for employee use at all times. There is a coffee carafe blocking access to the hand washing sink. Operator removed the carafe of coffee.
