Quaternary ammonium sanitizer not at proper minimum strength for manual warewashing. Do not use equipment/utensils not properly sanitized. three compartment sink sanitizer tested at 0ppm quaternary sanitizer. Manager made new solution, tested at 400ppm.
Hole in or other damage to wall under employee lockers and cove molding is disrepair around dish pit and by walking near back door.
Nonfood-contact surface soiled with grease, food debris, dirt, slime or dust. Grease/residue on exterior of bread bin. Cleaned during inspection.
