Utensils in poor condition. Pizza cutter has a build up of rust on screw holding cutting blade to handle. Pizza cutter was discarded.

Employee with ineffective hair restraint while engaging in food preparation. Employee wearing visor, however long hair is flowing over the shoulders and can cross contaminate food being prepared.

To order approved program food safety material, call DBPR contracted provider: Florida Restaurant and Lodging Association (SafeStaff) 866-372-7233. One employee, Mario, hired over 60 days without proof of training.
No proof provided that food employees are informed of their responsibility to report to the person in charge information about their health and activities related to foodborne illnesses. Operator had forms but they were not signed. Employees signed reporting agreement during inspection.
