Manager on duty, Tim, stated he has been a manager here for approximately six months and all three managers were going for training tomorrow. Manager stated Justice McDonald has been a manager for approximately one month and Jesse Fernandez has been a manager for approximately three months.
Bowl or other container with no handle used to dispense food. Plastic container without hand,e stored down inside bin of cornmeal at front counter. Manager removed pan from cornmeal during this inspection.
Ceiling tile missing. Most ceiling tiles in rear storage area missing. The ceiling tiles that are present are absorbent and porous acoustic tiles and should be smooth, nonabsorbent, and easily cleanable.
Dead roaches on premises. One dead roach on floor in rear storage room. Manager swept and discarded the dead roach during this inspection.
Nonfood-contact surface soiled with grease, food debris, dirt, slime or dust. Back-sides of monitors at front counter soiled with dust.
Unclean building components, attachments or fixtures. 1. Two air vents in kitchen at front counter soiled with dust. 2. Pipes and hoses on wall in kitchen beside pizza oven soiled with dust.

Employee touched soiled surface and then engaged in food preparation, handled clean equipment or utensils, or touched unwrapped single-service items without washing hands. Employee adjusted glasses and receded to prepare a pizza without washing hands. Inspector had employee stop and wash hands before continuing to prepare food.
Hose attached to faucet at three-compartment/food preparation/handwash sink eliminating required air gap. Spray nozzle hangs into sink below rim.

Rodent activity present as evidenced by rodent droppings found. Approximately 39 rodent droppings on floor in back dry storage room. Room is used to store two litter bottles of soda, wash laundry and store dirty dough racks. Manager began cleaning droppings.
Time/temperature control for safety food identified in the written procedure as a food held using time as a public health control has no time marking. Pizza sauce on prep table at cookline missing time mark. Manager stated the pizza sauce has been in-use since 11am and added proper time mark.
Vacuum breaker missing at mop sink faucet or on fitting/splitter added to mop sink faucet. Splitter added at mop sink. No device located on hose connection side. Hose is attached.
Food service manager not certified within 30 days of employment. Box cutting knife on shelf over sauce in walk in cooler. Two hand held brooms on shelf over clean dishes. Moved brushes during this inspection.
