Operator placed in walk in cooler to quick chill.

Vacuum breaker missing at mop sink faucet or on fitting/splitter added to mop sink faucet. Observed lack of vacuum breaker at mop sink with hose attached. Advised operator to purchase vacuum breaker or remove hose.

Plumbing system in disrepair. Observed Out of order sign on door to only bathroom. Per operator, tank for toilet not filling up and will not flush. Per operator, they fill up the toilet tank with water from mop sink in order to flush toilet.
