Quaternary ammonium sanitizer not at proper minimum strength for manual warewashing. Do not use equipment/utensils not properly sanitized. Triple sink reading 0ppm, employee states they have not changed out sanitizer solution since the morning and the dispenser over the triple sink reads at 300ppm quaternary ammonium.

Employee used the bathroom and then engaged in food preparation, handled clean equipment or utensils, or touched unwrapped single-service items without washing hands upon returning to employee area. Food service employee returned from bathroom and engaged in food service activity but did not wash hands second time. Manager intervened.

Wet mop not stored in a manner to allow the mop to dry. Wet mop stored on the corner of the mop sink not hung to dry. Operator relocated the wet mop.
