Observed multiple employees food safety training certificates expired. Operator able to provide one employee certificate that was not expired.
Equipment and utensils not properly air-dried - wet nesting. Observed clean stacked pans on storage shelf in warewashing unable to properly air dry
Wet mop not stored in a manner to allow the mop to dry. Observed wet mop stored in mop sink in a manner for mop not to properly dry.
Wiping cloth quaternary ammonium compound sanitizing solution not at proper minimum strength. Observed Sanitizer Bucket (Quaternary 0ppm) at prep station. Operator emptied bucket and remade sanitizer solution. Retested at 200 ppm

Water with a temperature of least 85 degrees Fahrenheit not provided/shut off at employee handwash sink. Hot water turned off at hand wash sink by dough station due to leak when turned off. Discussed utilizing opposite cook line hand wash sink until repaired.

Bathroom located inside establishment not completely enclosed with tight-fitting, self-closing doors. Self closer missing from front restroom.
Buildup of food debris/soil residue on equipment door handles. Door to walk in cooler with debris buildup.
Exterior door has a gap at the threshold that opens to the outside. Light visible under back door.
Water leaking from pipe and/or faucet/handle. Leak observed under triple sink. Person in charge placed catch bucket. Also, cold handle missing from hand wash sink by walk in. Also, caulking around hand wash sink by walk in with mold like substance. Not smooth and easily cleanable.
