Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit. Observed cooked pasta (49F - Cold Holding); cooked ham (47F - Cold Holding) in the lower half of a reach in cooler on the cook line. The operator stated the pasta had been held for approximately 3 hours. The operator moved the potentially hazardous foods to the walk in cooler.
Bowl or other container with no handle used to dispense food. Observed a plastic ramekin with no handle used as a scoop for granola in the kitchen. The operator removed the ramekin.
In-use utensil stored in standing water less than 135 degrees Fahrenheit. Observed in-use utensils stored in standing water behind the bar. The operator turned on the water for the dipperwell. Observed in-use utensils stored in 83F standing water at the front counter. The operator removed the utensils from the water.

Raw animal foods not properly separated from each other in holding unit based upon minimum required cooking temperature. Raw ground beef and beef muscle stored over raw fish and shrimp. Operator advised and corrected storage order.
Handwash sink used for purposes other than handwashing. Hand wash sink on cook line and back of kitchen used as dump sink. Operator advised and relayed to all staff to use hand wash sinks for hand washing only.
No soap provided at handwash sink. Located on end of cook line. Operator replaced.
Whole fruit displayed for self-service not wrapped and no utensils provided. Operator removed apples for rewashing and drying, and applying protective layer.

Accumulation of black/green mold-like substance in the interior of the ice machine/bin. Observed ice machine with a black/green mold-like substance.
A return visit. The problems from the previous inspection had been dealt with.

Observed establishment has been remodeled, dish room added, cooler/freezer modified with no plan review. Operator stated waiting on proper plans to submit.
