Vacuum breaker missing at the splitter added to mop sink faucet.
In-use utensil stored in standing water less than 135 degrees Fahrenheit. Observed an in-use mixing spoon stored in 88F standing water behind the front counter. The operator removed and properly stored the utensil.

Employee touched face/hair and then engaged in food preparation, handled clean equipment or utensils, or touched unwrapped single-service items without washing hands. Observed employee "itch" head above visor on hair with wrist and proceed to go back to work with same gloves on. After speaking with employee, employee removed gloves, washwash hands, and then went back to preparing food.
Food placed in soiled container/equipment. Observed on milk dispenser, on bottom of door, dried on liquid accumulation.
Handwash sink used for purposes other than handwashing. Observed handwashing sink at front counter (as soon as you walk behind counter) with drink stains inside showing its being used as a dump sink.
Nonfood-contact surface soiled with grease, food debris, dirt, slime or dust. Observed soiled gaskets throughout establishment on reach in coolers.
Wet mop not stored in a manner to allow the mop to dry. Observed wet mop stored in mop wringer and not properly hung up to air dry.
