Nonfood-contact surface soiled with grease, food debris, dirt, slime or dust. In walk-in cooler, vent cover, light cover, and surrounding areas with accumulated dust. Also gaskets on cook line cooler soiled with food debris, mold like substance.
Single-service articles improperly stored. Coffee filters uncovered over coffee machine at front line and drive thru window. Operator covered.

Single-service articles improperly stored. Boxes to go cups and lids stored on floor in hallway by walk-in cooler. Coffee filters stored uncovered on top of coffee machine by drive through window.
Wet mop not stored in a manner to allow the mop to dry. Two mops at mop sink not properly drying.

Employee rubbed hands together for less than 10-15 seconds while washing hands. Observed two employees in back kitchen near drive through window wash hands with less than 5 seconds of rubbing soap, spoke with both employees about proper handwash techniques, both rewashed hands correctly before returning to work.
Server handled soiled dishes or utensils and then picked up plated food, served food, or prepared a beverage without washing hands. Observed employee at front counter begin making a drink after picking up a sign from the floor. Discussed with employee that preparing drinks is handling food, and proper handwashing is required. Employee washed hands properly before returning to work.
Vacuum breaker missing at mop sink faucet or on fitting/splitter added to mop sink faucet. 2 way splitter added, missing one vacuum breaker.
No proof provided that food employees are informed of their responsibility to report to the person in charge information about their health and activities related to foodborne illnesses. Observed blank reporting agreement, manager reviewed agreement with employees at time of inspection and took down signatures on a chart after review.
