Handwash sink used for purposes other than handwashing. Observed handwash sink in front counter area used to store syrup bottles and cleaning brushes. Food Handler removed items at time of inspection.
No paper towels or mechanical hand drying device provided at handwash sink. Observed no paper towels at handwash sink in front counter area. Manager restocked paper towels at time of inspection.
Opened employee beverage container in a cold holding unit with food to be served to customers. Observed 2 employee beverage containers with lids on stored inside reach-in cooler which contains coffee creamer and refresher mix to be served to customers. Manager removed employee drinks during inspection.
Wet mop not stored in a manner to allow the mop to dry. Observed wet mop stored on floor. Manager hung mop to dry.

Ceiling not smooth, nonabsorbent and easily cleanable in food preparation, food storage, or warewashing areas. Plastic trash bag duct taped to ceiling cover missing tile area.
Ceiling tiles missing in renovation area where employees walk to access bathroom under exposed duct work, electrical wires, and insulation.
Observed electrical wiring in disrepair. For reporting purposes only. Wires hanging from dining room ceiling in path to bathroom. Foreman put wires higher up during inspection.
A return visit. The problems from the previous inspection had been dealt with.
Ordered shut on the spot. 5 violations recorded, 1 of them the tier the state says can make you ill. A reinspection is required before this kitchen may reopen.

Adjacent to the cookline make table is a construction hole measuring approximately 4 ft. x 1 ft. x 2 ft deep. There are 2 long handled shovels inside the hole. The make table with open lid contains cheese and tortillas. In-use spatulas are stored in the open air, on the make table, next to the hole and the dirt pile. Hot holding trays containing cooked eggs are not completely sealed when closed.
A minimum of one bathroom facility is not available for public use. Lobby and bathroom not open to customers due to renovations to establishment.
Floors not maintained smooth and durable. Large hole in floor by cookline, establishment installing drain.
Large pile of dirt by hole approximately 1 foot by 4 feet and 2 feet deep in floor next to make line refrigerator. Hole contains shovels used to dig out dirt.
Nonfood-contact surface soiled with grease, food debris, dirt, slime or dust. Dust/dirt on side of reach in cooler by hole.
