Observed employee picking up debris off floor, going to back to take a drink, returning to station not washing hands. Spoke with employee and employee washed hands.
Food placed in soiled container/equipment. Observed on top of creamer dispenser around spout dried creamer around spout.
Food stored on floor. Observed gallon milk container on walk in floor, and various items on freezer floor.

Self-closing metering faucet does not provide water for at least 15 seconds. Observed in ladies bathroom (automatic water to run and shut off ) water shutting off after 3 seconds causing inadequate handwashing procedures.
Single-service articles improperly stored. Observed a case of donut boxes stored on floor in dry storage room. Observed operator moving case to shelf
Single-service articles not stored inverted or protected from contamination. Observed coffee filters not covered properly. Observed operator putting a lid on coffee filter container.
Working containers of food removed from original container not identified by common name. Observed sugar container not labeled with name of item in container. Observed operator labeling container correctly.

Handwash sink used for purposes other than handwashing. Observed hand sink being used as a dump sink. Observed manager going to dump a drink in hand sink. Stopped Manager from doing so and explained that a hand sink is for hand washing only.
Bowl or other container with no handle used to dispense food. Observed employee using a cup and scooping ice. Spoke with manager. Manager spoke with employee.
Employee chewing gum while preparing food or while in a food prep/food storage/warewashing area. Observed employee chewing gum while making drinks at front counter.
Equipment in poor repair. Observed gaskets torn at make line for reach in drawers.
Floor area(s) covered with standing water. Observed at front counter, floor very wet and not mopped to dry or any wet floor signs out.
Ice buildup in reach-in freezer and/or walk-in freezer. Observed ice build up all around out side and inside of walk in freezer.
Wet mop not stored in a manner to allow the mop to dry. Observed wet mop stored inside mop bucket,not stored to dry properly.
