Establishment has no written procedures for employees to follow in response to a vomiting or diarrheal event where the vomit or diarrhea is discharged onto surfaces in the establishment. Owner printed the procedure before the end of the inspection.
No paper towels or mechanical hand drying device provided at handwash sink. Hand washing sink next to the three compartment sink. Manager placed paper towels at the hand washing sink.
A return visit. The problems from the previous inspection had been dealt with.
Ordered shut on the spot. 5 violations recorded, 2 of them the tier the state says can make you ill. A reinspection is required before this kitchen may reopen.

Employee personal items stored in or above a food preparation area, food, clean equipment and utensils, or single-service items. Employee phones stored on preparation areas in the front counter area. Employee removed the phones.
Floor soiled/has accumulation of debris. Floors soiled under all equipment in the front counter area.
In-use utensil stored in standing water less than 135 degrees Fahrenheit. Stirring spoons stored in standing water in the front counter area. Employee removed the spoons to be sanitized.

Observed employee use their phone and then touch clean equipment without washing their hands. Observed employee moving a trash can and then touch clean utensils without washing hands. Instructed the employees that they needed to wash their hands. Employees washed hands and changed gloves.

Observed approximately 40 rodent droppings under the preparation tables in the serving/ front counter area. Observed approximately 30 rodent droppings under the single service storage racks in the dry storage area. Observed approximately 20 rodent droppings under the chemical storage rack in the dry storage area.
Toxic substance/chemical improperly stored. Observed a spray bottle containing cleaning fluid stored next to the syrup pumps on the front counter. Employee removed spray bottle. As a repeat violation the importance of following proper protocol with this violation was reinforced with the employee.
Vacuum breaker missing at mop sink faucet or on fitting/splitter added to mop sink faucet. Observed no vacuum breaker attached to the splitter at the mop sink.
Certified Food Manager or person in charge lacks knowledge of foodborne illnesses and symptoms of illness that would prevent an employee from working with food, clean equipment and utensils, and single-service items. Person in charge had no knowledge of the BIG 6 Foodborne illness and they were not posted anywhere. Emailed the PIC a copy of the BIG 6 sheet.

Accumulation of black/green mold-like substance in the interior of the ice machine/bin. Observed interior of ice machine with pink mold like build up.

Observed employee phones and chargers stored on the front counter next to food service items and the hand wash sink. Observed employee shoes stored with drinks in the back dry storage shelf. Observed employee bag stored on the food prep table. Employees removed the items listed above.

In-use utensil in non-time/temperature control for safety food not stored with handle above top of food within a closed container. Observed scoop with a handle not stored above food in the sugar under the back prep table. Operator removed scoop.
Nonfood-contact surface soiled with grease, food debris, dirt, slime or dust. Observed food debris build up on the exterior of the milk, sugar, and espresso machines on the front counter area. Observed dirt and slime build up on the exterior of the ice bin in the front counter area.
Reach-in cooler interior/shelves have accumulation of soil residues. Observed build up of food residue in the bottom of the standing reach in cooler in the back storage area. As a repeat violation the importance of following proper protocol with this violation was reinforced with the employee.
