No proof provided that food employees are informed of their responsibility to report to the person in charge information about their health and activities related to foodborne illnesses. Two employees hired less than 60 days did not have employee responsibility agreement signed. Emailed DBPR Form HR 5030-103 to manager who had both employees sign and date.
