The handwash sink was used for purposes other than handwashing. Two lids were found in the handwash sink in the prep area. The operator removed the lids from the sink.
3 vents in the dining area are soiled with accumulated of dust.
Accumulation of mold-like substance in the interior of the ice bin by the manager office.
Dead roaches on premises. 1 dead roach, by the manager office. The operator swept up the dead roach and sanitized the area.
Nonfood-contact surfaces were observed to be soiled with food debris beneath and around the donut holder located at the front of the establishment. Dust accumulation was also noted on the ceiling near the fans inside the walk-in cooler. The employee cleaned the affected areas within the walk-in cooler during the inspection.
