Employee handled soiled equipment or utensils and then engaged in food preparation, handled clean equipment or utensils, or touched unwrapped single-service items without washing hands. Main kitchen- observed employee enter kitchen, removed ice scoop from ice machine, placed utensils in 3 compartment sink, returned to front counter and make coffee and packaged ready to eat donuts for customers without washing hands and putting on gloves.
