Quaternary ammonium sanitizer used to sanitize clean in place equipment not at proper minimum strength. Do not use equipment not properly sanitized. Observed sanitizer bucket reading 0ppm. Talked to manager and waiting on company to do maintenance on system. Manager added more sanitizing solution to bucket and read 200 ppm.
Handwash sink used for purposes other than handwashing. Observed handwashing sink by hot food prep station with trash and ice.
Floor soiled/has accumulation of debris. Observed floors in storage areas with debris.
Ice buildup in reach-in freezer and/or walk-in freezer. Observed ice buildup around interior and exterior door of walk in freezer. Also ice buildup around fans in freezer.

Reach-in cooler interior/shelves have accumulation of soil residues. Reach-in cooler next to drive thru area, bottom interior soiled.
Sanitizer bucket stored with food, equipment, utensils, linens, single-service, or single-use articles. At drink line, sanitizer bucket stored on counter at coffee taps. Manager moved bucket.
Wiping cloth quaternary ammonium compound sanitizing solution not at proper minimum strength. At drink station, Sanitizer Bucket (Quaternary 0ppm); Manager made new bucket, Sanitizer Bucket (Quaternary 200ppm)
