Observed an employee pick up paper from the kitchen floor, dispose of it in the trash, and then handle RTE food without washing their hands. Discussed and coached employee in proper methods. Employee washed hands and put a new pair of gloves.

Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit. Cream Cheese (56F - Cold Holding); Butter (45F - Cold Holding)
Handwash sink used for purposes other than handwashing. Observed employee discard ice from a cup into the hand wash sink across from prep line.
Soap dispenser at handwash sink not working/unable to dispense soap. Observed handwash sink in men's restroom no able to properly dispense soap.
Accumulation of black/green mold-like substance in the interior of the ice machine/bin. Observed mold like substance in ice machine in kitchen area.
Cold holding equipment not maintained in good repair. Do not store time/temperature control for safety food in this unit until the unit is repaired. Observed front line reach in cooler not cold holding properly. As per operator cooler went down approximately two hours prior to inspection. Operator put in service call and placed pans of food on ice for temperature control. Ambient temperature of unit is at 72F Observed employees walking to nearby cooler at front counter to keep items at proper temperatureto make orders.
Employee personal items stored in or above a food preparation area, food, clean equipment and utensils, or single-service items. Observed personal yogurt in walk in cooler next to cooked eggs. Observed employee purse on a shelf with wraps.
Nonfood-contact surface soiled with grease, food debris, dirt, slime or dust. Observed table setup next to drive thru soiled with dust on top. Observed above toasters hood system built up grease like substance.
