Observed employee walk orders outside to vehicle and walk back in put on gloves to engage in food preparation without first washing hands after touching soiled door. Educated employee and employee washed hands.
Vacuum breaker missing at hose bibb or on fitting/splitter added to hose bibb. Observed vacuum breaker on wrong side of split. Operator switch to correct side.
No paper towels or mechanical hand drying device provided at handwash sink. Observed no paper towels at front counter hand wash sink. Operator refilled.
Floor not cleaned when the least amount of food is exposed. Observed behind and underneath equipment soiled with debris.
