Accumulation of black/green mold-like substance in the interior of the ice machine/bin. Observed pink and black mold like substance/debris in the ice machine.
Ceiling/ceiling tiles/vents soiled with accumulated food debris, grease, dust, or mold-like substance. Observed dust build up on the ceiling tile and upper walls in the back storage area.
Employee personal items stored in or above a food preparation area, food, clean equipment and utensils, or single-service items. Observed an employee cellphone and keys on the back prep table. Removed.
Equipment and utensils not properly air-dried - wet nesting in the dish drying area.

Employee beverage container on a food preparation table or over/next to clean equipment/utensils. Observed two employee drinks on a prep table next to the oven. Manager removed beverages and stored correctly.
In-use utensil stored in standing water less than 135 degrees Fahrenheit. Observed spoons used to stir drinks in standing water temping at 73F. Manager removed and replaced with an empty container.

Ten (10) foot-candles of light at least 30 inches from floor not provided in walk-in freezer. Observed two light bulbs that were out in the walk in freezer. Manager stated that someone will be out to replace them later today.
