Ice scoop handle in contact with ice. Ice scoop at front counter with scoop handle in contact with the ice. Operator relocated.
Nonfood-contact surface soiled with grease, food debris, dirt, slime or dust. Dairy dispenser at front counter with debris build up on plate around nozzles.

Utensils in poor condition. On shelf opposite three compartment sink, clean plastic container with cracked edges of container.

Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit. In cookline reach in cooler, sausage (56F - Cold Holding); cheese (54F - Cold Holding); cream cheese (51F - Cold Holding); egg patty (46F - Cold Holding). Ambient air of cooler 37F, per operator observed cooler freezing over and turned off with product inside approximately 1 hour before hand. Operator removed all time/temperature control food for safety to walk in freezer and replaced with items from walk in cooler. At conclusion of inspection only sausage and cheese still above temperature range.
Required employee training provided by a third-party employee training program and operator unable to provide original certificate for employees trained. No original certificates for any food handler training, all photocopies.
