Vacuum breaker missing at mop sink faucet or on fitting/splitter added to mop sink faucet. Observed splitter added no vacuum breaker on hose connection at mop sink faucet.
Equipment in poor repair. Observed gaskets torn on small one door cooler next to hand sink by donuts display case. Ice machine side cover not installed, exposing cables and internal parts. Observed smoke detector hanging from ceiling above preparation area.

Employee cleaned table, used phone, then when to prepare a drink no handwashing observed. Manager coached employee on proper hand washing.
