Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit. Pan of falafel in reach in cooler at 44F. Employee states left out on the counter for about 10 minutes. Raw beef (74F - Cold Holding)-36F 20 minutes later; chicken (50F/47F - Cold Holding)-43F 20 minutes later in large tubs in prep area at room temperature. Operator states all meat placed under thaw around 10:40-11:00 this morning(approximately 3.5 hours), operator moved all meat to reach in freezer. Temperatures 20 minutes later indicated above.

Multiple certificates appear to have the name, birthdate and date of training/expiration scratched off or whited out/covered and new information written. Certificate for Devran B. has last name of Mendez still visible on certificate.
In-use utensil in non-time/temperature control for safety food not stored with handle above top of food within a closed container. Handle for rice scoop laying in rice. Employee moved to proper storage.
Time/temperature control for safety food thawed in an improper manner. Completely thawed large tub of beef and two large tubs of raw chicken at room temperature. Employee states all meat pulled from water that meat was thawing in shortly before inspection began. All products above 41F indicating improper thaw method being used.
