Establishment has no approved Alternative Operating Procedure. An employee placed cut lemon on a plate for service with their bare hands. Educated the employee on proper handling of ready to eat foods.

Raw animal food stored in same container as ready-to-eat food. In the walk-in cooler raw ground beef is stored over cooked black beans. Operator moved the ground beef to proper storage.

Ready-to-eat, time/temperature control for safety food prepared onsite and held more than 24 hours not properly date marked. There is chili in the walk-in cooker with no date marking. Per the operator the chili was prepared three days ago.

No proof provided that food employees are informed of their responsibility to report to the person in charge information about their health and activities related to foodborne illnesses. Inspector provided the operator with copies of the employee health reporting agreement, operator had the employees onsite read and sign the agreement.
Raw/undercooked animal food offered and establishment has no written consumer advisory. Raw animal foods must be fully cooked prior to service. Inspector provided the operator with a copy if the consumer advisory. Operator placed the consumer advisory in plain view in the dining room.
