Ready-to-eat, time/temperature control for safety food prepared onsite and held more than 24 hours not properly date marked. Observed multiple food items prepared on site in walk-in cooler with no date markings. Per person in charge food items were prepared within last 3 days.

Observed multiple commercially processed fish items thawing in original roe diced oxygen packaging in sliding reach in cooler drawers under cook line. Both kinds of fish items have label stating to remove from packaging.
Food stored on floor. Observed multiple food items stored on floor in walk-in cooler and walk-in freezer.

Employee removed phone. Also, employee cell phone on metal rack at end of cook line across from stove. Cell phone on shelf above condiments and next to pack of rolls. Employee removed cell phone.
Floor, wall and/or ceiling soiled/has accumulation of debris. Floor under oven and fryers on cook line soiled with food debris/grease. Also, floor under steam table soiled with black build up. Also, floor under prep table/food wash sink soiled with food debris. Also, edge of wall/ceiling in back hallway with walk-in freezer/walk-in cooler soiled with mold-like substance.
In-use tongs stored on equipment door handle between uses. On cook line, tongs hanging on handle of stove door. Employee removed tongs from handle and stored differently.

Chef stated they have been in cooler since last night and voluntarily discarded. In cook line flip top reach in cooler sliced cheese (49F - Cold Holding).
