Per operator butter was placed on time after being softened in microwave. Educated operator on proper use of time as a public health control. Operator discarded butter.

Employee failed to wash hands before putting on gloves to initiate a task working with food. Observed team member handle cell phone and then proceed to place on new gloves to handle food. Had operator to have team member to discard gloves, wash hands and place on new gloves.
