In steam table cooked steak (133F - Hot Holding) per operator steak prepared on flat top stove and hot held in steam table, discussed with operator hot holding, operator moved product back to flat top to reheat to 187F.

In reach in cooler cut lettuce (47F - Cooling) at 9:20 since 9:00 to 47F at 9:40, at this current rate of cooling product will not reach 41F within 4 hours, discussed with operator to move to another cooler to allow for faster cooling.
Handwash sink not accessible for employee use at all times. Hand washing sink filled with dishes and non accessible, discussed with operator who cleared sink.

Dead roaches on premises. In kitchen approximately 3 dead roaches under three compartment sink. In kitchen approximately 2 dead roaches under dry dry storage rack, discussed with operator to clean and sanitize areas.
Food stored on floor. In kitchen large pots of cooked chicken, cooked cow tongue, bag of lettuce and multiple sauce bottles, discussed with operator to remove products from floor.
Plumbing system in disrepair. Exterior water drain leaking water on to ground, discussed with operator to repair.

Roach activity present as evidenced by live roaches found. Approximately 1 live roach fell from ceiling onto inspectors iPad, inspector had to sanitize iPad using alcohol swaps. Approximately 2 live roaches walking on floor in between steam table and flip top cooler. Approximately 30 live roaches under three compartment sink next to waste water tank. Discussed with operator to clean and sanitize all areas and eliminate all roaches.
Time/temperature control for safety food, other than whole meat roast, hot held at less than 135 degrees Fahrenheit. In steam table refried beans (129F - Hot Holding) per operator product prepared on stove top and moved to steam table approximately 1 hour ago, discussed with operator who reheated product to 172F and placed back on steam table.
