Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit. Reach in cooler under stove top in cook line area- shrimp 57f/53f, chicken 57f, beef 57f, fish 57f, imitation crab "krab" 57f/52f, chorizo 57f/Cook line prep cooler left side of cook line- diced tomatoes 50f, shredded lettuce 52f, pico 48f- door opened frequently during lunch and operator placed ice on food items of concern to bring temperatures down.
Commercially processed reduced oxygen packaged fish bearing a label indicating that it is to remain frozen until time of use no longer frozen and not removed from reduced oxygen package. Upon inspection observed reduced oxygen packaged fish with label indicating that it is to remain frozen until time of use no longer frozen without being removed or package punctured to allow air for thawing. Operator punctured package during inspection.

In-use tongs stored on equipment door handle between uses. Upon inspection observed tongs on oven handle. Operator removed tongs during inspection.

Employee began working with food, handling clean equipment or utensils, or touching unwrapped single-service items without first washing hands. Employee wearing gloves exited the back door to outside then returned to cook line to prepare food items without washing hands/changing gloves. Manager took steps to ensure proper handwashing methods were utilized.
