Nonfood-grade hose conveying potable water. A green nonfood-grade hose is used to collect clean water. Discussed with operator to obtain proper food-grade safe hoses.
Food-contact surface soiled with food debris, mold-like substance or slime. Multiple cutting boards inside food truck are soiled.
Self-sufficient mobile food dispensing vehicle operator failed to submit a Commissary Notification form to the division for each commissary prior to use.
